CoPMRE Eleventh Annual Symposium: Impact in Healthcare Research and Education’

The Centre of Postgraduate Medical Research and Education are hosting their Eleventh Annual Symposium on Tuesday 14 October 2014.

The event will focus on developments and activities around impact in healthcare research and education. It will explore impact from the perspectives of the public, the research funder, the university, the provider, the student and the medical educator.

Speakers include:

Dr Terry Haines, Monash University, Melbourne.

Reversing research and implementation science for practices that are widely provided, dogma heavy and evidence light.

Some widely provided health services have an absence of evidence for effectiveness, cost-effectiveness and/or safety yet persist in clinical practice. It is possible that these practices are wasting valuable resources, but alternately may be valuable assets to service provision. Provision of these services in the context of usual care is a considerable barrier to conducting a conventional trial. Our team has recently developed a novel research approach to conduct a trial for this context[1]. This approach turns a conventional stepped-wedge, cluster randomised controlled trial design on its head.  This presentation will outline the strengths and limitations of the stepped-wedge design relative to other experimental designs, describes how this design was turned into a novel disinvestment research design, and then describe its first application in a clinical setting. The clinical example involves the withdrawal of weekend allied health services from acute medical and surgical wards across three hospitals in Australia. The early results of this trial run contrary to current initiatives to create a 7-day a week health service.

“Obesity Prevention in Men” with Professor Edwin van Teijlingen

On July 2nd Prof. Edwin van Teijlingen will present findings from a HTA report published this month.  Researchers from the University of Aberdeen, Bournemouth University and the University of Stirling examined the evidence for managing obesity in men and investigated how to engage men with obesity services. The evidence came from trials, interviews with men, reports of studies from the UK, and economic studies.

 

The research found that men are more likely than women to benefit if physical activity is part of a weight-loss programme.   Also eating less produces more weight loss than physical activity on its own.  However, the type of reducing diet did not appear to affect long-term weight loss.

 

Prof. van Teijlingen will highlight some of the key messages for Public Health policy and practice.  For example, that although fewer men than women joined weight-loss programmes, once recruited they were less likely to drop out than women.   The perception of having a health problem, the impact of weight loss on health problems, and the desire to improve personal appearance without looking too thin were motivators for weight loss amongst men.

This work has been funded as part of the ROMEO project (Review Of Men and Obesity) by the National Institute for Health Research, Health Technology Assessment Programme (NIHR HTA Project 09/127/01).

Dr Terry Haines from Melbourne will be in London for a conference and has offered to visit BU on Tuesday 8th July.

He will give a presentation at 2pm in TA134, Talbot campus, entitled:

“Researching health services that have an absence of evidence but are already a part of standard care”.

Terry is a distinguished researcher, with a physio / economics background, expertise in both quali and quant methods, and has 17 PhD students doing work around inpatient falls prevention as well as a variety of other topics.

BUDI presents at the 2014 meeting of the North Sea European dementia group

 

I recently had the pleasure of attending the 2014 meeting of the North Sea European dementia group, held in Dijon, France. This was an unusual meeting in that it was not a large formal scientific conference attracting only researchers, but a small and informal meeting that attracted researchers, practitioners, private care directors, and a research funder. Around 25 were in attendance and over three days we got to know each other, exchange ideas, and make important connections for potential future pan-European collaborations.

Days 1 and 2 – the meeting

The first two days was a series of presentations with discussion. This began with a representative of each European country providing an introduction of their organisation and an update on work in progress. We then had a series of presentations, and of note, some was of work that was just starting or half way through, and some presented service improvement initiatives yet to be evaluated using research methodology. This was good as it afforded the sharing of ideas at their initial stages when changes can be made, rather than at the end when already completed. I presented a qualitative paper on our NIHR-funded project on the social care and support needs of people with dementia and sight loss. This was well received and as suspected, not been given much consideration before. It led to an interesting discussion around diagnosis, given that tests to indicate dementia symptoms often rely on good vision (e.g. to the clock drawing task).

Perhaps the most useful aspect of these two days, during the presentations and informal discussions over meals, was the sharing of what current dementia care looks like in each other’s countries, the limitations to such care, and the challenges and opportunities that lie ahead in driving forward improvement. The mix of researchers and those directly involved in managing services provided a useful forum for the identification of the challenges and opportunities to working together. In particular, to make sure that research is grounded in the real-world constraints practitioners work within, and to make sure that the novel initiatives practitioners try out are formally and systematically evaluated in order to demonstrate benefit to people with dementia and cost-effectiveness. In particular, fidelity of interventions was raised as an issue, in ensuring that those who claim to deliver best practice actually do deliver it and in the way specified.

Day 3 – the visits

Before heading back, we spent the last morning visiting local services for older people with dementia. I was with the group that visited a day care centre and an intergenerational area. The day centre was a relatively large space and so caters for up to 20 older people per day, much more than the average day centre. They had several rooms for different activities (rest room, computer room, physical activity room, etc.), along with a garden. This was on the same site as a care home, and so for those that eventually need to move into a care home, the day centre provides a nice source of continuity for those that become residents. It was pleasing to see a lot of the creative work that the older people there as this was on display all over. One display that caught my eye was one of the older people helping children with their art work.

We then went on to an initiative that is rare in France and one that I had not come across before. Over ten years ago a plot of land was allocated for providing services for people in the city. At the time it was decided that the same space be used not only for catering for older people to live independently but with support (like sheltered housing), but also to provide for children under five (pre-school) and social housing for the general adult population who need it. In this same plot we got to see the living spaces for older people with dementia, social housing, and could see the pre-school right next to it. I unfortunately had to cut this visit short to head back for my flight, but it was great to see an initiative that at its heart was trying to re-connect the generations. We know from classic theories of the psychology of ageing that intergenerational relationships provides benefits for all, whereby the older person shares their wisdom and perspective to the benefit of the younger person, let alone connection with everyday family life. It would be interesting to see what activities best bring these groups together to facilitate social interactions between the generations.

In sum, the meeting was a useful learning experience of what is happening in other European countries and provided an important set of connections for potential future collaborations. I can recommend attending such meetings, even if they do not have the prestige of large scientific conferences. The more intimate and informal environment was useful for learning about work in its early stages and to learn more about the contexts in which our European collaborators operate within.

Next Friday the 2nd of May at 14:00 h in PG10, we will have a research seminar in neuroscience entitled “Emergent oscillatory activity in the cerebral cortex”.

Our guest is Prof. Maria Victoria Sanchez-Vives, http://www.sanchez-vives.org/,  ICREA Research Professor at the IDIBAPS (Institut d’Investigacions Biomediques August Pi i Sunyer) in Barcelona, head of the Systems Neuroscience group.

Prof. Maria Victoria Sanchez-Vives has published a number of influential papers in journals like e.g. Science, Nature Neuroscience or PNAS and is currently the Chief Editor of Frontiers in Systems Neuroscience. She has been funded by Human Frontier Science Program, national and international agencies and has been partner in six European Projects. She is currently coordinator of the FET EU project CORTICONIC.

Her main interests include how neuronal and synaptic properties as well as connectivity determine the emergent activity generated by neuronal networks. The integration of the cortical information giving rise to bodily representation and the combination of brain-computer interfaces and virtual reality for understanding these processes, is another research line of her group.

We strongly suggest not to miss the opportunity to attend to this seminar. Afternoon cakes, coffee and tea will be served during the event.

Title: “Emergent oscillatory activity in the cerebral cortex”.

Abstract: “Understanding complex systems like brain networks is a challenge. Cortical networks can perform computations of remarkable complexity, accounting for a large variety of behaviours and cognitive states. At the same time, the same networks can engage in stereotypical patterns of spatio-temporal activation, such as the ones that can be observed during sleep, anaesthesia and in cortical slice. Collective phenomena emerging from activity reverberation in cortical circuits at different spatio-temporal scales results in a rich variety of dynamical states. Slow (around or below 1 Hz) and fast (15-100 Hz) rhythms are spontaneously generated by the cortical network and propagate or synchronize populations across the cortex. This is the case even in isolated pieces of the cortical network, or in vitro maintained cortical slices, where both slow and fast oscillations are also spontaneously generated. The similarity between some of these patterns both in vivo and in vitro suggests that they are somehow a default activity from the cortical network. We understand that these emergent patterns provide information on the structure, dynamics and function of the underlying cortical network and their alterations in neurological diseases reveal the circuits dysfunction”.

 

 

 

Seminars, hospitals, falls, and a cuddle with a koala

Earlier this month I was invited back to Australia for another research visit to Monash University. I was invited to visit Melbourne last year for a research visit and to present at a one-day seminar. This was along with two other UK colleagues from Birmingham and York, who also specialise in falls prevention (one is a geriatrician and the other a nurse). The three of us were invited back and this time we presented at two one-day seminars.

The seminars

Day 1: The first day was focused on falls prevention among older people. I was invited to give two presentations, one on an area of long-term interest to me; increasing older people’s uptake and adherence to falls prevention interventions. The other was to present findings on a pilot project funded by all the RCUK’s and departments of health (led by the MRC), on the topic of outdoor falls. This led to some interesting discussions, and it was also useful to hear other presentations from leading researchers from Sydney.

 

Day 2: The second day focused on patient safety. Some challenging issues were discussed such as learning lessons from mid-Staffs. There was also some discussion of preventive medicine, and I was invited to lead a workshop on behaviour change techniques to increase participation in physical activity interventions for the prevention of falls.

 

Research visit

Before the two one-day seminars, we had some engaging discussions sharing about the research being conducted at our respective institutions. This included work that will lead to an international position paper on how to advance the science of falls prevention research. We also visited a local hospital whereby we were invited for a Question Time type meeting, with the three of us UK visitors plus our host (Dr Anna Barker, Monash University) were quizzed on falls prevention in the hospital setting(!). This proved stimulating and interesting that the same issues being tackled over here are similar to what is experienced at the other side of the world. We also had the excitement of the first participant to be recruited into the RESPOND trial in which I am a collaborator. The RESPOND trial is a multi-site randomised controlled trial that will test the effectiveness of a patient-centred programme to prevent secondary falls in older people presenting to the emergency department with a fall. The protocol paper should be published in the near future…

 

And the koala…

Before heading back to the UK I managed to get 24 hours in Adelaide to visit a colleague who visited the UK back in around 2007. We’ve kept in touch over the years as her PhD was on a similar theme to mine around the psychological barriers to older people’s participation in falls prevention interventions. We managed to get to a zoo and queue up for the much awaited cuddle with a koala. This was fab; like cuddling a teddy bear that is alive! My hosts also kindly arranged for a fish and chip meal on the beach before catching the flight home (felt a terribly British thing to do!).

 

On the whole this was a good visit to do. It led to several useful discussions that I have taken back with me that will shape my research direction, both in the immediate future with my current grant proposal writing and subsequent proposals. It was also useful to strengthen the collaboration between our institutions and engage in useful dialogue with practitioners about the issues I as a researcher am trying to help them overcome.

Bournemouth European Network in Cyber Security (BENICS)

In recent years, the field of Cybersecurity has attracted researchers and practitioners from academic fields ranging from Computer Science and Design, through to Psychology and Business Studies. To date, however, these communities have not been influenced by each other. Their research are disseminated in a variety of workshops and conferences across these fields. As a result, there is a misunderstanding of the role these different fields play in improving cybersecurity. For example, some researchers describe people are “the weakest link” and encourage designers to build systems that “Homer Simpson” can use safely. Unfortunately, treating users as a problem limits opportunities for innovation when people are engaged as part of a solution. Similarly, treating practitioners like cartoon characters disenfranchises the very people that a design is meant to support. Bournemouth University is one of the few institutions in the world with interests across the disciplines contributing to Cybersecurity, a small enough size for academics across these disciplines to engage with each other, and the vision necessary to fuel this engagement. To take advantage of the opportunities afforded to Bournemouth, an interdisciplinary seminar series in cybersecurity was launched in September 2013. The seminar series has attracted both staff and students from across the university, together with practitioners from local industry with interests in cybersecurity. So far, this has led to connections forming across the Faculty of Science & Technology, and the Media and Business schools. Resulting collaborations with our seminar speakers have also led to prospective KTP and Horizon 2020 proposals, and invitations to deliver guest lectures at other universities.

To build on this momentum in interdisciplinary cybersecurity activity at Bournemouth, we have created the Bournemouth European Network for Interdisciplinary Cyber Security (BENICS): a FUSION funded SMN activity. Over the coming year, BENICS will bring five invited European cybersecurity academics to Bournemouth to engage in short (one-week), focused collaborative visits. These visits will introduce invited academics to Bournemouth’s cybersecurity capabilities, allow them to share their interests with us as part of the cybersecurity seminar series, and engage in short and focused proposal building, research, or teaching resource creation activities.

Following each visit, Bournemouth and the visiting academic will engage in pump-priming activities; these will refine deliverables produced to sustain the momentum created during the visit. These deliverables will form the basis of a joint publication at an agreed international conference or journal.

-The first of the seminars of this series will take place next Wednesday the 12th of March, 15:00, P302 LT. The invited speaker is Dr Dimitris Pinotsis, http://iris.ucl.ac.uk/iris/browse/profile?upi=DPINO08.
Dr Pintosis obtained his PhD in September 2006 from the Department of Applied Mathematics and Theoretical Physics (DAMTP) of the University of Cambridge. After an EPRSC Research Fellowship and lectureship in Reading University he moved to UCL where he is working at the Welcome Trust Centre for Neuroimaging ; having secured funding from EPSRC and the Wellcome Trust.
Dr Pinotsis has a strong track record and a number of landmark publications in imaging neuroscience modelling; he is also the author of the most advanced versions of the state-of the art models for neuroimaging data, the dynamic causal models. I am familiar with Dimitris work and I very strongly encourage the attendance to researchers both in machine learning and in cognitive psychology.
The title of his exciting talk is “Electrophysiological Data and the Biophysical Modelling of Local Cortical Circuits”. “Dynamic Causal Modelling (DCM) is a general framework that allows for a formal (Bayesian) analysis of the properties of neuronal populations, based upon realistic biophysical models. In the past few years, a wide variety of such models has been implemented in the DCM framework. In this talk, I will first review some of these recent advances and then focus on models that allow one to infer spatial parameters of cortical infrastructures generating electrophysiological signals (like the extent of lateral connections and the intrinsic conduction speed of signal propagation on the cortex). I will try to highlight the links between different models and address how the experimental hypothesis or question asked might inform the choice of an appropriate model”.

Maria V. Victoria Sánchez-Vives, M.D., PhD in Neurosciences has been ICREA Research Professor at the IDIBAPS (Institut d’Investigacions Biomediques August Pi i Sunyer) in Barcelona since 2008, where she is the head of the Systems Neuroscience group. She is currently co-director of the Event Lab (Experimental Virtual Envir onments in Neuroscience and Technology).
After obtaining her PhD at the University of Alicante in Spain, MVSV was postdoctoral fellow/research associate at Rockefeller University (1993-1994) and Yale University (1995-2000). She next established her own laboratory at the Neuroscience Institute of Alicante (Consejo Superior de Investigaciones Científicas) while being Associate Professor of Physiology. Her independent research has been supported by national and international agencies. She has been funded by Human Frontier Science Program and has been partner in six European Projects. She is currently coordinator of the FET EU project CORTICONIC.

Health, Well-Being & Society: New Wordle

The members of the Health, Wellbeing & Society theme have responded enthusiastically to the call to provide the five keys word reflecting their research interests and expertise.  The new Wordle above was created Feb. 13th from all of those key words.  Apart from presenting a pretty picture of colourful words, the Wordle provides a quick overview of the kind of research conducted by BU staff and PhD students affiliated with the theme.

Workshop by Dr Falko Sniehotta entitled “Behaviour change techniques to promote healthy lifestyles”

Dr Sniehotta is visiting BU to provide staff and postgraduate students the opportunity to participate in a workshop on behaviour change techniques.  The session will give colleagues a flavour of the kinds of techniques one might use to help people make changes to adopt and sustain healthy lifestyles.  After an introduction, the workshop will mainly be spent giving colleagues hands on experience with testing out some of the evidence-based techniques currently in use, and will finish with a presentation from Dr Sniehotta on his current research.  There will be some preparation required before this event of reading  journal articles that  Dr Sniehotta will provide.

Dr Falko Sniehotta is a behavioural scientist. His research programme aims at developing and testing a) theory of behaviour change and b) interventions to change behaviours relevant to health and health care. This research is conducted with his colleages in the IHS, the Newcastle Health Psychology Group and colleages nationally and internationally.
He is president of the European Health Psychology Society, Associate Editor of Health Psychology Review, and member of the editorial boards of the Annals of Behavioral Medicine, Psychology & Health and the British Journal of Health Psychology. His post is currently funded by Fuse, the UK CRC Centre for Tranlsational Research in Public Health and their research is funded by the Medical Research Council, the National Institute of Health Research and other funders.

Scanning Brains, Bones, and Artefacts: Magnetic Resonance Imaging at Poole Hospital

For the past two years I have made very slow progress in attempting to convince Poole Hospital to open access to their MRI scanners for research purposes.  Whilst I had originally responded to an email from them it seems there were not at all ready to deal with my requests. More recently there has been some positive movement on the issue.  I am hoping that this technology might interest you.  Poole Hospital has three scanners of two field strengths: two at 1.5T and one at 3T, the latter being the standard for neuroimaging, but the former being of use for high-resolution structural scans of people and objects.  The applications for this type of technology are many; in psychological research it is used most commonly to get brain scans of patients or to measure brain activity as people perform tasks, but has been used effectively as an analytical tool in Archeology and Sports Science; you will know better than I how this technology has been used in your fields.  I am trying to gauge the level of interest in this technology at BU so as to make a better case  to BU and to the hospital.  Please contact me on bparris@bournemouth.ac.uk if there is utility in the use of this technology in your research or teaching or if there could be in the future.
