Migration and mental health meeting in Kathmandu

Yesterday (29th December) Prof. Edwin van Teijlingen attended a workshop on ‘Current situation of migration and its impact on Mental health’ in a hotel in Kathmandu, Nepal. This workshop was organised by the Transcultural Psychosocial Organization Nepal (TPO Nepal) and funded by a Finnish aid organisation. Two of BU’s current collaborators were members of the workshop panel. One of the panel members was FHSS Visiting Faculty Prof. Padam Simkhada, who is Professor of Global Health at the University of Huddersfield. He spoke about the limited amount of research into mental health in general in Nepal and in migrant workers in particular. The chair was Mrs. Manju Gurung from the organisation POURAKHI Nepal, who collaborates with BU in the recently started project on health and migration funded by the USA State Department. Unsurprisingly, several speakers at the workshop referred to studies conducted in the field of migrant workers’ health by BU academics, particularly the work published by Dr. Pramod Regmi and Dr. Nirmal Aryal.

Methods paper: Online questionnaire surveys

The paper ‘Guide to the design and application of online questionnaire surveys‘ [1] led by Dr. Pramod Regmi, Principal Academic in International Health in the Faculty of Health & Social Sciences, reached 200 citations today on ResearchGate. Our methods paper was published in 2016 in the Nepal Journal of Epidemiology which is Open Access, hence it is freely available to anyone in the world with internet access.

At the time conducting online surveys was still a bit of a novelty. In the paper we noted that there was limited literature to help a fledgling researcher with the design and a use of online questionnaires. Therefore, this short paper highlights issues around: a) methodological aspect of online questionnaire survey; b) online survey planning and management; and c) ethical concerns that may arise while using this option.

Promoting interdisciplinary research

This week’s Bournemouth University (BU) advertised a forthcoming event in February 2024 ‘Early Career Researchers and Interdisciplinarity in the Medical Humanities – South West‘. To promote this event I would like to highlight some papers published recently by BU academics on interdisciplinarity.

First, Dr. Shanti Shanker in the Department of Psychology and Dr. Pramod Regmi in the Department of Nursing Science and Prof. Edwin van Teijlingen in the Department of Midwifery & Health Sciences published the paper ‘The Interdisciplinary Team Not the Interdisciplinarist: Reflections on Interdisciplinary Research’ [1]. This interesting paper is co-authored with two BU Visiting Faculty, Ms. Jillian Ireland, Professional Midwifery Advocate in Poole Maternity Hospital (University Hospitals Dorset NHS Foundation Trust) and Prof. Padam Simkhada based at the University of Huddersfield. This paper argues that interdisciplinary working is within the team, not the person being interdisciplinary . Multidisciplinary teams provide unique opportunities for researchers from different disciplines (and hence different ways of working and thinking) to collaborate with one another. We need to be careful not to try to create interdisciplinarists, or at least, not too many. The worlds needs people who are strong in their discipline and open-minded/flexible/reflective enough to see the value of other people’s disciplines.

Secondly, the Journal of Manmohan Memorial Institute of Health Sciences published our editorial ‘Public Health is truly interdisciplinary’ [2]. This editorial was largely written to counteract some of the jurisdictional claims made in Nepal by certain people in Public Health. These claims express themselves in arguments around the question whether Public Health is a single academic discipline or a broad interdisciplinary profession comprising many different individual academic disciplines. There are two quite distinct and opposing views. Some argue that Public Health is a broad-ranging single discipline covering sub-disciplines such as Epidemiology, Management, Medicine, Nursing, Health Psychology, Medical Statistics, Sociology of Health & Illness and Public Health Medicine. Those who support this argument, typically see: (a) Public Health is the overarching dominant discipline, which brings these sub-disciplines together; and (b) that a true Public Health practitioner amalgamates all these individual elements. Others argue that Public Health is more an overarching world view or interdisciplinary approach for wide-ranging group of professionals and academics [2]. In this view some Public Health professionals are first trained as clinicians, others as psychologists, health economists, health management, statisticians, or demographers, and so on and have later specialised in Public Health.

Thirdly, doing multidisciplinary research is not without its problems, hence we reflect on some of these in the paper ‘Interdisciplinary Research in Public Health: Not quite straightforward’ [3]. The authors are BU’s Dr. Pramod Regmi, Dr. Nirmal Ayral in the Department of Nursing Science and Prof. Edwin van Teijlingen, and BU Visiting Professor Padam Simkhada and BU graduate Dr. Pratik Adhikary (currently at the University of the West of England). We all are Public Health researchers, with very different educational backgrounds and training, reflecting the diversity of and interdisciplinarity in the field. Several of us have a first degree in Education or Health Education, but one has a first degree in Sociology. Whilst four of the five authors have Master degree in Public Health and/or Health Promotion, two have a Master in Education. Most of the authors have a Ph.D. in Public Health, but again the Ph.D. of one of us is in Sociology. The key point is that we are all working in Public Health.

Unfortunately the ‘Early Career Researchers and Interdisciplinarity in the Medical Humanities – South West’is for ECRs from the South West Hub only.

New publication on positionality

Today the journal Health Prospect published Raksha Thapa’s paper ‘Researching Dalits and health care: Considering positionality’ [1]. This methodological paper is based on Dr. Raksha Thapa’s recently completed BU project on caste-based inequity in health care utilization in Nepal, particularly focusing on people at the bottom of the caste hierarchy, commonly known as Dalit communities. Her PhD study was supervised by Dr. Pramod Regmi, Principal Academic in the Department of Nursing Sciences, Prof. Vanessa Heaslip Professor of Nursing and Healthcare Equity at the University of Salford and Prof. Edwin van Teijlingen in the Centre for Midwifery & Women’s Health. This is the third paper from her PhD work; the first one was published in the Journal of B.P. Koirala Institute of Health Sciences [2] and the second one in the Asia Pacific Journal of Public Health [3].

Interview with BU’s Dr. Nirmal Aryal in main Nepalese newspaper

Dr. Nirmal Aryal, who is a part-time researcher in the Department of Nursing Sciences, was interviewed in Nepal by a journalist for Kantipur, the largest Nepali-language newspaper of the country. Nirmal was interviewed after our successful research dissemination events last week in both the capital Kathmandu and in Janakpur (in the south of Nepal). These dissemination programmes reported on a study into the risk of kidney damage in Nepalese migrant workers. This study, led by Bournemouth University’s Dr. Pramod Regmi,was funded by The Colt Foundation in the UK. Dr. Nirmal Aryal is also research associate in the Equality, Diversity and Inclusion Research Unit of Greater Manchester Mental Health NHS Foundation Trust.

Media coverage BU’s kidney research in Nepal

This week Bournemouth University organised two dissemination events for our risk of kidney disease study in Nepalese migrant workers in the Middle East and Malaysia. A previous blog reported on the first event in the capital Kathmandu (see details here!) . These dissemination events have generated a loads of media coverage in Nepal, both in Nepali and in English.

The study was led by Bournemouth University and a charity in Nepal which whom we have been collaborating for two decades, called Green Tara Nepal. This important study, the first of its kind, was conducted among the Nepalese migrant workers and a comparison group of non-migrants from the same community. This study was funded by The Colt Foundation, based in the UK. In the field it was supported by the Madhes Province Public Health Laboratory, the United Nations’ International Organization for Migration and University College London (UCL).

Dr Pramod Raj Regmi (Principal Academic in International Health in Bournemouth University’s Department of Nursing Sciences) is the lead researcher and our team further comprises researchers Dr Nirmal Aryal and Prof Edwin van Teijlingen (both from BU’s Faculty of Health & Social Sciences), and in Nepal clinicians: Prof Dr Arun Sedhai, Dr Radheshyam KC and Dr Shrawan Kumar Mishra.

Dissemination kidney-disease research Kathmandu

Yesterday, Sunday 8th October we held an event in the capital of Nepal to disseminate the findings of our study of the kidney health of Nepalese migrant workers working abroad. The study included 718 migrants and 725 non-migrants from Dhanusha district which has the highest number of labour migrants working abroad. Our study found that 5.8 % of our migrant samples had some sign of kidney injury compared to non-migrants (3.6%). The study also reported other lifestyle risk factors in migrants than non migrants. Labour migration has become an integral part of Nepali society, over a quarter of the country’s national income is from remunerations, i.e. workers sending money home from abroad. It is therefore important to measure and record these problems related to kidney health to get policymakers and other relevant stakeholders to implement culturally adapted and feasible interventions to promote healthy lifestyle and improve working conditions.

This mixed-methods study adopted Disadvantaged Populations eGFR Epidemiology Study (DEGREE) protocol which combines a questionnaire around living and working conditions abroad with biological measurements. This study, funded by the UK-based Colt Foundation, is the first of its kind in Nepal. The BU team comprises Dr. Pramod Regmi, Principal Academic and Dr. Nirmal Aryal, Postdoctoral Researcher, both in the Department of Nursing Science, and Prof. Edwin van Teijlingen, in the Department of Midwifery & Health Sciences. This event yesterday in Hotel Radisson in Kathmandu was first of two dissemination events, the second one will on Wednesday 11 October in the fieldwork area. In Kathmandu some 45-50 people attended including on of the regional ministers of Labour, Employment & Transport.

Two papers based on ERASMUS+ exchange

In the latest issue of the Journal of Manmohan Memorial Institute of Health Sciences appeared two articles focusing on the ERASMUS+ programme. This new issue of the journal was published earlier this month, it is an Open Access journal hence its articles can be read free of charged. Unfortunately, due the UK leaving the European Union (EU), ERASMUS+ has just come to an end this summer for universities in the UK.

The first article is an editorial that outlines the benefits of the staff and student exchange between Manmohan Memorial Institute of Health Sciences (MMIHS) in Nepal and Bournemouth University [1]. The paper concludes that the world is rapidly becoming a smaller place with globalisation occurring everywhere. The process of internationalising higher education institutions through partnerships like ours, brings benefits to both MMIHS and BU and to both staff and students. For the latter, it offers new ideas, alternative ways of thinking, insights in the other people’s value systems and cultures, etc. It equips students and staff with knowledge, skills and dispositions to work in culturally diverse and international contexts, providing them with a wider diversity of knowledge and greater understanding of global issues and challenges. In short, the partnership offers many opportunities to gain new experiences, explore new sources and perspectives, and improve their cross-cultural capabilities and, ultimately, their employability.

Secondly, there is an article co-authored by the seven MMIHS students, who came to BU in late 2022-early 2023, highlighting what they had learnt from one of the modules they attended [2]. This paper has as it central feature involving patients and the general public in all aspects of research, which was very much a new idea for these seven MSc students from Nepal.

BU top in health-related research in Nepal

A few days ago a group of researchers from the UK, Nepal and South Korea listed as the top five authors writing on migrant workers’ health-related research in Nepal. Their recent paper ‘Migrant workers’ health-related research in Nepal: A bibliometric study’ was published last week in the journal Dialogues in Health (Elsevier) with as lead author Dr. Sharada P. Wasti from the University of Greenwich [1]. The team conducted a systematic search of published literature on Nepalese migrant workers’ health was conducted in Scopus, Medline, CINAHL, Embase, PsycINFO and Web of Science, followed by a bibliometric analysis. The search of databases retrieved 520 records, and a total of 161 papers were included in the analysis. Bibliometric analyses were performed to create visualisation maps. The team found that most articles were published on infectious diseases, followed by health and lifestyle, sexual and reproductive health, access to health services, workplace safety, maternal health, and health systems and policy.

Their search found that 533 researchers originating from 24 countries contributed to the pool of literature. It also lists the top five authors in the field in Table 5. We were proud to find out that all five top author positions are affiliated with Bournemouth University (BU): three are current BU staff, Dr. Pratik Adhikari one is a BU PhD graduate, and Prof. Padam Simkhada from the University of Huddersfield, is BU Visiting Faculty.

New BU migration and health publication

Congratulations to Drs. Pramod Regmi and Nirmal Aryal in the Faculty of Health & Social Sciences who published their latest paper this week [1]. This peer-reviewed paper ‘Assessing the knowledge of, attitudes towards, and practices in, food safety among migrant workers in Klang Valley, Malaysia’ in the journal Travel Medicine & Infectious Disease assesses the current food safety KAP (knowledge, attitudes and behaviour) as well as strategies to promote food safety awareness, among migrant workers across occupational sectors in Malaysia.

The authors conducted a survey with 403 migrant workers using telephone interviews and online self-administered questionnaires. The respondents were Nepalese, Filipino and Indonesian migrant workers. The majority were male, working in the services industry, had completed high school, aged between 30 and 39 years and had worked in Malaysia for less than ten years. Knowledge was significantly correlated with attitudes and practices. Female respondents had lower knowledge and attitude scores while younger respondents had lower knowledge scores. Indonesian and Filipino respondents had lower knowledge and attitudes scores than Nepalese respondents. Understanding food safety information from social media was positively correlated with the respondents’ food safety knowledge and practices. The paper concludes there is: (i) a to target female, younger, Indonesian and Filipino migrant workers, and (ii) the potential of social media to improve public awareness of food safety and hygienic practices.

Reflections from visiting Erasmus+ students to Bournemouth University

Late last week my colleague Dr. Pramod Regmi, Senior Lecturer in International Health, returned home from his Erasmus+ exchange to Nepal. He brought home for me a copy of the MMIHS (Manmohan Memorial Institute of Health Sciences) Souvenir 2023, this outliens key events at the institution of the past year. One of the write ups in the Souvenir is from the seven MMIHS M.Sc. students in Public Health who visited Bournemouth University in late 2022-early 2023 for three months. The students have since all completed their M.Sc. in Public Health at MMIHS. Their story highlights some of the British features of student life which were new to them. These included the support they received from our SUBU (=students’ union), or registering with the NHS, and the UNIBUS app, as well as cultural celebrations during their time in Dorset, such Halloween, Christmas and New Year. They rave about the module Public Involvement in Research and specifically thank Dr. Mel Hughes and BU’s PIER (Public Involvement in Education & Research) team. Their second module at BU focused on Systematic Reviewing, they particularly mention the guidance and support received from Prof. Vanora Hundley in the Centre for Midwifery & Women’s Health (CMWH) and our Faculty of Health & Social Sciences librarian Mr. Caspian Dugdale.

Unfortunately, the Erasmus+ scheme has come to an end, but Bournemouth University has just been awarded funding for a serious number of Turing Scheme exchanges. The latter is for BU students to go aboard, and this funding supports study exchanges, work placements, voluntary traineeships and other international extra-curricular or curricular activities worldwide for a minimum of 28 days and maximum of 12 months.

BU team meets with Green Tara Nepal in Kathmandu

A team of academics from Bournemouth University (BU) on the Erasmus+ exchange met with the Non-Governmental Organisation (NGO) Green Tara Nepal (GTN) in Kathmandu this week. BU has been collaborating with Green Tara Nepal for nearly 15 years. GTN is non-profit making charity, established in 2007, which focuses on research, publications, advocacy, training and social mobilization in a holistic community development approach. GTN works closely among the individuals and groups through health promotion using participatory and community development approaches. Over the years various BU academics have conducted many research projects with the support of GTN in Nepal, organised dissemination events and conferences, wrote grant applications, published papers, and so so.

The BU Erasmus+ visitors included FHSS academics Prof. Steve Ersser, Dr. Michele Board and Dr. Pramod Regmi, as well as the Head of BU International Partnerships Dr. Alistair Morrison and Global Engagement Officer Elaine Hakes, and from the department of Psychology Dr. Shanti Shanker, and last, but not least, FHSS Ph.D. student Yagya Adhikari.

I would personally like to say thank you very much for all the support I have received from GTN over the past two decades!

Global Consortium in Public Health meets in Huddersfield

On Monday and Tuesday 18-19 June the University of Huddersfield will organize its Global Consortium in Public Health meeting. This meeting is the brain child of Prof. Padam Simkhada, he is Visiting Professor at Bournemouth University and based at the University of Huddersfield. The event brings together public health researchers and experts from the UK, the USA, Ghana, Nepal, India, Qatar and Brazil to discuss the latest developments and challenges in the field. The Global Consortium in Public Health is an international network of public health researchers, practitioners, and policymakers who are committed to advancing the field of public health through collaborative research, education, and advocacy. The consortium provides a platform for sharing best practices and building future collaborations.

On Monday 19th June Prof. Edwin van Teijlingen will be talking about the REF 2028 and the importance of strong international partnerships in the fields of research and education. BU’s Dr. Pramod Regmi was also invited to this event in Huddersfield, but he is on his way to Nepal as part of Bournemouth University’s Erasmus+ staff and student exchange with Manmohan Memorial Institute of Health Sciences (MMIHS).

BU PhD student publishes in The Conversation

Congratulations the Abier Hamidi, PhD student in the Faculty of Health & Social Sciences (FHSS) whose PhD work was published in The Conversation this week (24 April) under the title Social media now trumps traditional family networks in Libya – my Facebook survey reached 446,000 women. Her piece in The Conversation on the recruitment of female participants for a PhD study in a rather patriarchal society brings together issues of anonymity, gender, and wider social culture.

This is Abier’s PhD research is supervised by Dr. Pramod Regmi, Senior Lecturer in International Health and the Global Engagement Lead in the Department of Nursing Sciences, and Prof. Edwin van Teijlingen in the Centre for Midwifery, Maternal & Perinatal Health (CMMPH).

Several BU papers at this week’s BNAC Nepal Study Days

The BNAC (Britain-Nepal Academic Council) twentieth Nepal Study Days (24-25 April) start today in Huddersfield. BU is directly represented through Dr. Pramod Regmi who will be presenting ‘Health Research Priorities and Needs in Migrant Health: A Review of Reviews’ today. Dr. Regmi based in the Department of Nursing Sciences and Prof. Edwin van Teijlingen in the Department of Midwifery & Health Sciences are also part of the paper ‘Nepal Family Cohort Study: Starting a Longitudinal Population Health Study’ which will be presented by our colleague Dr. Om P. Kurmi from Coventry University.

There will also be a BU paper on ‘Perceptions Around Covid-19 and Vaccine Hesitancy: A Qualitative Study Among People Living in Kaski, Western Nepal’ , presented by Dr. Preeti Mahato. Preeti is based at Royal Holloway (University of London) and she is also a Visiting Faculty member of FHSS. She’ll presenting a study recently published in PLOS Global Public Health.

These Nepal Studies Days pay serious attention to our ongoing interdisciplinary study ‘The impact of federalisation on Nepal’s health system: a longitudinal analysis’ which was reported in several national newspapers in Nepal last week. Tomorrow there will be a plenary session by Prof. Simon Rushton, the PI on this project in which BU collaborates. He will be talking about ‘Nepal’s Health System During Federalisation: Emerging Issues and the Co-production of Policy and Practice Recommendations’. Congratulations to Abier Hamidi on the acceptance of her Ph.D. paper ‘Facilitators and barriers to condom use in Middle East and North Africa: a systematic review’. [1] This review has been registered on PROSPERO. [2] The Journal of Public Health is part of BU’s publishing deal with Springer, hence it will free open access when published.

Abier is supervised by Dr. Pramod Regmi, Senior Lecturer in International Health and the Global Engagement Lead in the Department of Nursing Sciences, and Prof. Edwin van Teijlingen in the Centre for Midwifery, Maternal & Perinatal Health (CMMPH). Earlier Abier published ‘HIV epidemic in Libya: Identifying gaps’ in 2021. [3]

Setting up a longitudinal health & lifestyle study in Nepal

The Nepal Family Cohort Study (NeFCoS) is a multidisciplinary cohort study into the prevalence, incidence, and determinants of various diseases from childhood to adulthood in children and their parents for over two decades. NeFCoS is a longitudinal study which recently started collecting health and well-being data, and which will collect data repeatedly from the same individuals at certain intervals for years to come. Our primary focus is understanding the multiple risk factors of health conditions in children and their families, early diagnosis of diseases, and prevention and management of diseases through effective intervention. The team aims to this by examining associations between exposure to known or suspected causes of disease, so-called risk factors and subsequent morbidity and mortality.

Our large international team is led by Dr. Om Kurmi, Associate Professor and Respiratory and Environmental Epidemiologist at Coventry University in the UK, and associated with McMaster University, Canada. The Bournemouth University team comprises Dr. Pramod Regmi (Senior Lecturer in International Health), Dr. Edwin van Teijlingen (Professor of Reproductive Health), and Dr. Vanora Hundley (Professor of Midwifery).

The team is currently recruiting six-to-nine-year-old children and their parents who have provided written consent to participate. The parents offer information on socio-demographics, lifestyle factors, dietary habits, occupational history, educational history, environmental conditions at home and outside, physical activities and any diseases they or their children have had since birth. They also undergo a series of measurements such as lung function, oxygen saturation, blood pressure, hand-grip strength, anthropometry, body fat percentage, muscle mass, body water content and skin-fold thickness as an indirect measure for malnutrition. The study is also measuring eleven different parameters in spot urine samples semi-quantitatively and plan to collect the biological specimen in future follow-ups studies. The study is currently being conducted in two regions of Nepal in the south, the Terai and in the more central hill part of the country, with plans to expand to extend to other provinces of Nepal in the future.

The baseline study is ongoing. So far, the team has collected over 4250 participants’ data from both study sites. We plan to complete the baseline data by the end of December 2023 with about 15,000 participants from the two regions. Our study is urgently needed in Nepal as it will provide valuable information for evidence-based decision-making regarding disease prevention and management along with changing policy.

Congratulations to Dr. Pramod Regmi for his appointment this month as Visiting Faculty at Manmohan Memorial Institute of Health Sciences (MMIHS). Dr. Regmi is Senior Lecturer in International Health as well as Global Engagement Lead in the Department of Nursing Sciences. He is also REF Outputs Champion for Unit of Assessment 3 ‘Allied Health Professions, Dentistry, Nursing, Midwifery & Pharmacy’.

Bournemouth University has currently a staff-student exchange with MMIHS which is funded under the European Union’s Erasmus+ programme. Whilst BU and MMIHS also have a partnership agreement which we are currently in the process of renewing.
