International midwifery collaboration on early labour

Academics from the Centre for Midwifery and Women’s Health (CMWH) traveled to Winterhur, Switzerland with the Swiss European Mobility Programme.

The education and research programme, organised by the team at Zürcher Hochschule für Angewandte Wissenschaften (Zurich University of Applied Sciences), involved a workshop with midwives from across Europe ( Switzerland, Denmark, Sweden and Germany) discussing midwife-led care.

Professor Vanora Hundley and Dr Dominique Mylod were also invited to give a lecture on the latent phase / early labour as part of the final Gebstart conference. They presented recently published research that was included in thespecial issue in Women & Birth at the end of last year.

This meeting and collaboration with colleagues from across Europe builds on the work of the International Early Labour Research Group.

Appreciative Inquiry Resource

Today we received a copy of the book Appreciating Health and Care in the post. This book has a sub-title ‘A practical appreciative inquiry resource for the health and social care sector’ and refers to the work led by Bournemouth University’s Dr. Rachel Arnold. Appreciative Inquiry values people’s expertise and vision and can motivate people to see the world differently and instigate positive change. Rachel been the lead author on several publications around Appreciative Inquiry [1-3].

Writing some blurb

Publisher Routledge announced the forthcoming edited volume Menstruation in Nepal: Dignity Without Danger, which is edited by Sara Parker, Madhusudan Subedi and Kay Standing. This book examines the complexities of menstrual beliefs and practices in Nepal. Taking an interdisciplinary and intersectional approach, it explores and promotes the rights of women, girls and people who menstruate, to a dignified and healthy menstruation. I had the honour of being asked to write some of the blurb for this exciting book. Partly, because of our wide-range of health services and health promotion research in the country and partly because of our previous paper on reusable sanitary towels in the aftermath of the 2015 earthquake in Nepal [1].

Collaborative midwifery paper cited 40 times

Two days ago ResearchGate informed us that that the paper ‘Midwifery-led antenatal care models: mapping a systematic review to an evidence-based quality framework to identify key components and characteristics of care‘ has reached 40 citations. This paper, co-authored by Bournemouth University’s Professors Vanora Hundley and Edwin van Teijlingen, was originally published in 2016 in BMC Pregnancy & Childbirth [1]. Both Vanora and Edwin are based in the Centre for Midwifery & Women’s Health (CMWH) in the Faculty of Health & Social Sciences.

The same team wrote a separate paper the following year on ‘Antenatal care trial interventions: a systematic scoping review and taxonomy development of care models’ [2]. Interestingly, ResearchGate tells us this paper has been read fewer times and cited ‘only’ 21 times to date.

New BU Public Health publication

Yesterday we received the proofs of the recently accepted paper ‘Improved Water, Sanitation and Hygiene Facilities at School and their Effect on Educational Achievement in Basic Level Students in Nepal’ [1]. Luckily these are only the proof pages as my family name is misspelt, and the paper still lists the old name of our Centre for Midwifery and Women’s Health, which was, of course, the correct name at the time of submission.

The last time I published a paper on hygiene was also with colleagues in Nepal seven years ago, this time it was on menstrual hygiene [2].

Paper by BU MSc student accepted for publication

Congratulations to Mr. Musa Lewis Nhlabatsi whose paper ‘Clinicians’ barriers to screening and diagnosing diabetes distress in patients with type 1 and 2 Diabetes Mellitus: a systematic review’ has just been accepted by the African Journal of Health Sciences [1].This systematic review’s initial search identified 1,579 studies, but only four primary studies from three countries met the inclusion criteria. The studies reported five barriers: (1) lack of knowledge, (2) lack of time, (3) lack of accessibility to mental health services, (4) lack of motivation and (5) patients’ denial of their diabetes distress. The two most reported barriers were lack of knowledge and time. In conclusion this review identifies critical barriers to the underdiagnosis of diabetes distress by clinicians and highlights the need for policymakers and organisations to conduct pragmatic research to understand clinicians’ experiences in assessing diabetes distress in various healthcare settings to improve diabetes management.

The paper developed from his work on the Systematic Review Module for Masters student. Earlier Musa won The Department of Medical Science and Public Health Award in July 2023 and he graduated in November 2023.

Health Promotion article is being read

Our article ‘Understanding health education, health promotion & public health’ [1] is getting read according to ResearchGate. This conceptual/ theoretical paper was published open access in late 2021 in the Journal of Health Promotion and it reached 4,500 reads yesterday. Whilst the web side of the journal suggests today that the PDF of the paper has been downloaded 8,511 times.

New paper by FHSS PhD student Abier Hamidi

This morning the journal Discover Social Science & Health informed us that Abier Hamidi’s latest paper ‘Islamic Perspectives on HIV: A Scoping Review’ has been accepted for publication [1]. Discover Social Science & Health is an Open Access journal publishing research across the full range of disciplines at the intersection of health, social and biomedical sciences. This latest review is part of Abier’s PhD research project and it follows several earlier related publications [2-7].

Abier is supervised by Dr. Pramod Regmi, Principal Academic-International Health and the Global Engagement Lead in the Department of Nursing Sciences, and Prof. Edwin van Teijlingen in the Centre for Midwifery & Women’s Health (CMWH).

A tale of two ministries

Yesterday in Nepal we attempted to meet two government ministers to present policy recommendations generated by the study examining the consequences for the health system of Nepal’s move to a federal government structure. This three-year project is UK-funded by the MRC, Wellcome Trust and the FCDO (Foreign, Commonwealth & Development Office; formerly DFID) under the Health Systems Research Initiative. Through our collaborating partners in Nepal we had managed to make initial contacts with the Prime Minister’s office and a senior secretary in the Ministry of Health & Population. The timing was significant as yesterday was the last day that UK-based members of our team were in Nepal. The day started with a possible meeting with first the Prime Minister Pushpa Dahal, followed by a possible meeting with the Health Minister Mohan Baladur Basnet. However, as the saying goes ‘timing is everything!’ And, in the end the time was against us.

One of the two principle investigators of our study, Professor Simon Rushton from the University of Sheffield, and got ready early in the morning, only to be told by our Nepalese collaborators that the traffic in Kathmandu was hectic. Having waited for nearly an hour for our transport, we decided to split up with one of us joining some of our Nepalese collaborators trying to see the Prime Minister (PM) and the other one with a team to visit the Minister of Health & Population. Prof. Rushton went to the latter and after a period of waiting he and one of our Nepalese collaborators managed to meet the Minister and his staff. He also managed to hand over our policy recommendations. This offered a nice picture opportunity to illustrate a REF Impact Case Study.

Three other Nepalese collaborators and I, on the other hand, waited for over two hours in the waiting room of the Prime Minister, who turned out to be in an urgent meeting with two senior ministers. After over two hours we had to call off our attempt to see the PM. All I ended up with as ‘evidence’ was a picture of the tissue box in the PM’s waiting room. In the end I had to rush to Tribhuven International Airport to catch my flight to Qatar, then onward to London Gatwick.

For the team visiting the PM it felt very much like Robert Burns’ poem: “The best laid schemes o’ Mice an’ Men. Gang aft agley.”

Research methods capacity building in Nepal

Yesterday, Sunday 14th January, I was invited by Bournemouth University’s former PhD student Dr. Pratik Adhikary to run a three-hour methods session on semi-structured interviews and focus group discussions at the Nepal Injury Research Centre (NIRC). The workshop was based on work normally presented as part of BU’s Doctoral College Researcher Development Programme.

The audience comprised PhD students based at NIRC, as well as at Kathmandu Medical College (KMC), and Nepal’s oldest and largest university, Tribhuvan University. Participants were involved in research into many different research questions, ranging from road traffic accidents to sexual health and the well-being of migrant workers. NIRC was established with funding from the UK National Institute for Health and Care Research (NIHR) Global Health Research Programme and it is a partnership between KMC and the University of the West of England, Bristol (UWE).

TV interview in Nepal

Two days ago Professor Edwin van Teijlingen from BU’s Faculty of Health & Social Sciences was interviewed by the national television channel News24 in Kathmandu, Nepal. His interview covered some more personal questions, including the question how Edwin became involved in research in Nepal two decades ago, followed questions about recent research projects. The latter involved questions about the nearly completed project “The Impact of Federalisation on Nepal’s Health System: A longitudinal Analysis’. This project, launched in 2020 at the beginning of the COVID-19 pandemic, is led by the University of Sheffield, in collaboration with BU, the University of Huddersfield, Canterbury Christ Church University, PHASE Nepal and MMIHS (Manmohan Memorial Institute of Health Sciences). This nearly four-year project is UK-funded by the MRC (Medical Research Council), the Wellcome Trust and DFID (now called the Foreign, Commonwealth & Development Office [FCDO]) under the Health Systems Research Initiative.

There were also questions about the health and well-being of Nepal’s migrant workers, partly related to a recently finished study on kidney health of migrant workers funded by The Colt Foundation and a new project led by La Isla Network in the United States of America (USA). La Isla Network, Johns Hopkins University Bloomberg School of Public Health (in the USA) , Nepal Development Society and Bournemouth University are leading the first-ever international effort to research and address trafficking among Nepalese labour migrants. The work is funded by a $4 million cooperative agreement awarded by the U.S. Department of State’s Office to Monitor and Combat Trafficking in Persons, International Programs.

Communication in the aftermath of an NHS staff suicide: New research published today!

Prof. Ann Luce (FMC), Ms. Georgia Turner (PhD candidate FST), Ms. Lauren Kennedy (MSc student FST) and Dr. Reece D. Bush-Evans (Lecturer in FST) are pleased to announce the publication of their most recent work in British Medical Journal: Medical Humanities titled, “Quite simply they don’t communicate: a case study of a National Health Service response to staff suicide”. You can access the article here for free.

Workplace suicide can have significant knock-on effects within an organisation, yet research has shown within the healthcare profession, not all staff receive suicide prevention training, and few employers take the time to reflect on the need to change workplace policies or practices following the death of a staff member to suicide. How staff suicide is communicated across an organisation and to family members is important. Effective crisis communication is critical for effective management for a timely and sensitive response to a staff suicide within an organisation. By doing so, workplaces can help to reduce the significant emotional trauma suicide can have on an employee, and support good mental health across its workforce.

This groundbreaking work in the field of suicide prevention is already having an impact. The work was cited by Parliament’s Public Accounts Committee recent report on “Improving Mental Health Services”. Furthermore, the research has served as the underpinning evidence for NHS England’s National Suicide Prevention Toolkit for England, which will be implemented across all NHS Trusts in England. And, with a renewed focus on healthcare suicide, with a specific focus on female nurses, the research served as underpinning evidence for England’s National Suicide Prevention Strategy that was released in September 2023.

The team would like to thank all research participants as this was a difficult project to complete. Further, thanks must also be extended to NHS England for funding, the BU Open Access Fund, and the colleagues across BU who read drafts of the work prior to publication.

Latest paper from Federalisation & Health System in Nepal

The sixth paper from our interdisciplinary research team focuses on the effective way we applied participatory policy analysis in a study on the effects on the health system in Nepal. In the research we used a methodological approach using the River of Life which we describe in this paper ‘Participatory policy analysis in health policy and systems research: reflections from a study in Nepal’ [1]. The Lead author is Dr. Sujata Sapkota from Manmohan Memorial Institute of Health Sciences (MMIHS) in Kathmandu.

This study was funded by the UK Health Systems Research Initiative [Grant ref. MR/T023554/1]. In this larger Nepal Federal Health System Project we study the consequences for the health system of Nepal’s move from a centralised political system to a more federal government structure in 2015. This joint project is led by the University of Sheffield in collaboration with Bournemouth University, the University of Huddersfield, Canter Bury Christ Church University and two institutions in Nepal, namely MMIHS and PHASE Nepal. We have managed to publish five papers from this project prior to today’s one reflecting on the methodology [2-6].

Forum on Post-Federalisation Health System Strengthening

Today and tomorrow our research team is engaged in discussions with those responsible for running the health system at different levels of the new federal system in Nepal. The aim today and tomorrow of this participatory research project is bring together stakeholders from all levels of government (local, provincial and federal), to develop solutions, practical actions and recommendations for different levels of the political system to address some of the five areas our research identified as possible priorities. Nepal changed from a centralised political system of government to a federal system in 2015. It is easy to see how such change in the political system might affect the organisation, funding, governance, human resources, etc. of all sub-systems in society, such as the education system, the police, and in the area of our particular interest, the health system.

This interdisciplinary study started just before COVID-19 in 2020 and is now coming to a conclusion. The multi-national research team includes researchers from Nepal: MMIHS (Manmohan Memorial Institute of Sciences) in Kathmandu, and PHASE Nepal (Bhaktapur), the University of Sheffield, Bournemouth University, and the University of Huddersfield (the original UK applicants), and researchers currently based at three further UK universities: the University of Greenwich, the University of Essex and Canterbury Christ Church University. This exciting research is funded by the Health System Research Initiative, a UK collaboration between three funders: the MRC (Medical Research Council), the Foreign, Commonwealth & Development Office (FCDO), and the Welcome Trust.

First of all: Happy New Year!

One of the first message I received this morning was that our editorial ‘Addressing the inequalities in global genetic studies for the advancement of Genetic Epidemiology’ [1] had been published yesterday. If I had know this in time it would have been the proper last Bournemouth University Research Blog of 2023 published yesterday. Interestingly, we only submitted the draft editorial on Christmas Day, got it back for revisions on Boxing Day and resubmitted it and had it accepted on December 28th. It dis, of course, help that both editors-in-chief of the Nepal Journal of Epidemiology are co-authors on this editorial!

Interview with BU professor published

Today Madhusudan Subedi and Man Bahadur Khattri published their Interview with Prof. Edwin van Teijlingen in the Dhaulagiri Journal of Sociology & Anthropology [1]. This academic journal is published in Nepal and it is Open Access; hence freely available to read to anybody with access to the internet.

Their abstract reads: “Edwin van Teijlingen is Dutch by birth and a Professor of Reproductive Health at Bournemouth University in the south of England. He has achieved a PhD in Medical Sociology at the University of Aberdeen, UK. Prof. Edwin van Teijlingen connected with Nepal while supervising Nepali PhD scholars in UK, and he has been a frequent visitor to Nepal since 2006. He has supervised more than 35 PhDs, among which 13 are Nepali. He has examined more than 50 PhDs. He has published around 300 academic papers and book chapters in health promotion, midwifery, and health services research. He serves as a peer reviewer for worldwide famous health journals such as PLOS One and BMC Pregnancy & Childbirth. He delivered a speech to the Members of Parliamentarians in Kathmandu in 2016 as part of a workshop to promote evidence-based policy-making. He is a committee member on various grant-awarding bodies in the UK, Belgium, the Netherlands, Norway, Switzerland, and the USA. He is a visiting Professor at the Centre for Disability Studies, Mahatma Gandhi University, Kerala in India (2020-present); the School of Health Sciences at the University of Nottingham in England (2017-2026); Nobel College, affiliated with Pokhara University, Nepal (2012-present); and Manmohan Memorial Institute of Health Sciences affiliated with Tribhuvan University (2009-present). We would like to express our gratitude for his acceptance to share personal and academic lives, which can inspire young and energetic scholars in Nepal and elsewhere.”

Paper published on ‘living evidence’

The Nepal Journal of Epidemiology published today carries an article on so-called ‘living evidence’ as an on-going synthesis approach that provides up-to-date rigorous research evidence [1]. This short research methods paper argues that living evidence is particularly useful in rapidly expanding research domains, uncertain existing evidence, and incorporating new research evidence that may impact policy or practice, ensuring that health worker, managers and health-policy makers have access to the best, i.e. the most recent evidence.

The Nepal Journal of Epidemiology is an Open Access journal, and hence freely available to researchers across the globe. The paper has been co-authored by researchers from the Denmark, Qatar, Mauritius and the UK.

New Nepal paper published

BMJ Global Health published our latest paper today under the title ‘The impact of decentralisation on health systems: a systematic review of reviews‘ [1]. This so-called review of reviews is part of a larger study funded by the UK Health Systems Research Initiative [Grant ref. MR/T023554/1]. in this larger Nepal Federal Health System Project we study the consequences for the health system of Nepal’s move from a centralised political system to a more federal government structure. This joint project is led by the University of Sheffield in collaboration with Bournemouth University, the University of Huddersfield, and two collaborating institutions in Nepal, namely Manmohan Memorial Institute of Health Sciences (MMIHS) and PHASE Nepal. We have managed to publish four papers from this project prior to today’s one. [2-5]

This latest paper, is Open Access and it includes nine reviews, each addressing somewhat different questions, contexts and issues. More negative than positive impacts of decentralisation on health system building blocks were identified. Although inconclusive, evidence suggested that the impacts on leadership and governance and financing components in particular shape the impact on the overall health system. Assessment of how the impact on building blocks translates to the broader impact on health systems is challenged by the dynamic complexities related to contexts, process and the health system itself. This review highlights some of the common potential issues to consider in advance.
